Initial Visit

During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. If possible, please print and complete these forms prior to arriving to your appointment so we can expedite your visit.

Please arrive early to ensure adequate time for check in. Please be sure to bring the following items:

• Patient’s insurance card
• List of current prescriptions and/or over-the-counter medication, including dose and frequency
• Information about patient’s medical and surgical history
• Recent test results, x-rays, or relevant records

Insurance Information

Our practice accepts most insurances. We will be more than happy to verify your insurance and available coverage when you call for your appointment.

Methods of Payment Accepted

Our practice accepts cash and all major credit cards. Checks will not be accepted for the first visit. We also have CareCredit financing for more expensive procedures
